Billing Policy

  • A deposit is due at the time of service for any of the following
    • insurance maximum out-of-pocket expense (including deductible) is not met.
    • after reasonable efforts, we are not able to verify insurance eligibility or estimate financial responsibility online.
  • Any copayment or coinsurance is due at the time of service.
  • After our office bills the insurance and receives the explanation of benefits statement, you will be refunded or billed based on the account balance and your financial responsibility.
  • Self-pay patients (without insurance or insurance we do not accept):  payment due at time of service
  • No shows and < 24 hour late cancellations are subject to a fee.
